When should I use this form? If you are planning a ward or branch event or outing that includes an overnight stay or travel inside the local area, complete this form and submit it to your bishop two or three weeks prior to advertising or promoting the event.
For outings that involves one or more high risk activities or that requires travel outside the local area, stake approval is required. For stake presidency approval, submit the following:
Event and Activity Plan
Activity Safety Plan
Event Itinerary (only for activities two or more days in length)

When should I use this form? For all activities involving one or more of the high-risk activities (aquatics; firearms; climbing, rappelling, or caving; off-road vehicles; archery; or horseback riding) or that require travel outside the local area, an Activity Safety Plan is required.
Submit the Activity Safety Plan with your Event and Activity Plan two to three weeks prior to advertising or promoting the activity.
When should I use this form?
Ward Application: Use this form for all activities and outings that take place outside of your ward boundaries, for all overnight activities, and for ward activities with higher than normal risks. Present form to ward youth leaders.
Stake Application: Use this form for all stake activities and outings that take place outside of our stake boundaries, involve an overnight stay, or for stake activities with higher than normal risks. Present form to stake youth leaders.
If an accident occurs, what should I do? Activity leaders should promptly notify the bishop and stake president if an accident, injury, or activity-related illness occurs on Church property or during a Church-sponsored activity. The bishop, stake president, or designated member with knowledge of the incident should promptly report the incident online incidents.ChurchofJesusChrist.org using the Global Incident Reporting system (GIR).
